# making the implicit explicit
One of my philosophies at work is to **make the implicit explicit**.
If there are only a few outcomes or options, it's probably worth outlining these in full.
People shouldn't have to guess, or to assume that something is "common sense", when practice may vary between institutions.
When discussing a patient in a MDM, you have a room full of people who have just heard the case in full and seen the imaging and pathology.
- "Repeat PET and re-discuss" is one option.
- But another option could be "Repeat PET. If demonstrates stable disease, proceed with original plan for definitive intent treatment. If demonstrates signs of distant metastatic disease, for referral to Medical Oncology for consideration of systemic therapy options."
